Transition
A partnership involving students with disabilities, families, school and post-school service personnel, local community representatives, and employers. All students leave school to transition into their adult lives. "Transition" describes that critical transformation and includes the education, legal information, planning, and community connections of our special education students need to achieve adult goals.

Who Should Attend?
Parents/Guardians/Family members who have a young adult with a disability, teachers, school staff, and others who are interested in helping students transition to life after school.
Registration
Please be sure to indicate which workshop(s) you plan to attend. You will receive a confirmation email, including a Zoom link to access the workshop. There must be a minimum of 5 attendees registered by the deadline date or the event will be canceled.
Accessing Services and Opportunities with Community Mental Health of Ottawa County

This informational session will focus on the criteria for eligibility for services with Community Mental Health of Ottawa County, as well as a discussion on the various Medicaid services covered for adults (18+ years and older). The resources presented will include those which assist adults with Intellectual and Developmental Disabilities (IDD) to live and participate in this community.
Supporting the Launch: Tips for Parents and Caregivers Supporting Teens and Young Adults with Transition to Adulthood

Having parents/guardians who have high expectations for their children, as well as knowledgeable and actively involved in the transition planning process, has been identified as an evidence-based predictor of improved post-school outcomes for students with disabilities (Mazzotti et al., 2015; Test et al., 2009). Devon Raap, School Social Worker at Ottawa Area Intermediate School District Young Adult Services, will provide tips on how parents/guardians and caregivers can support their teen and young adult by holding high expectations and providing opportunities in the home environment and community to learn and practice skills that support independence and adult living.
