Employee Injury/Accident (Workers' Compensation)
Please use these forms to report any work-related employee accident/incident, return completed forms to Human Resources.
The Advance Approval Form must be completed and signed by a supervisor, prior to beginning coursework. Upon course completion, the Tuition Reimbursement Form must be filled out and returned to Human Resources along with all required documentation. Tuition group guidelines and amounts are as follows:
OAEA,CTCTA, District Services, Behavior Specialists and Social Workers, SPS and JJI teachers and aides, CTC Para/aides, OAC and Infant occupational/physical therapy aides and nursing staff.
MTEC Open Certifcate Program Instructors, OSEA and Behavior Assistants
